• Your Full Name
• Full Address (if you wish to apply Gift Aid to your sponsorship or donation)
• Phone Number
• The amount you wish to pledge
• Indication of whether or not you wish to apply Gift Aid - If you pay UK tax and say 'yes' to Gift Aid, the Government will give us at least 25% on top of your donation. It won’t cost you a penny. Please give your full name and home address and say 'yes' or we can't claim Gift Aid.
